Form 3115 MTM Day Trader

Can someone please walk me thru step by step on how to fill out Form 3115 as a day trade? My 481a adjustment is zero.

I've read over a number of threads and am seeing mixed advice, some people are saying part ii Qs 5-12 is only for corporations other people are saying you need to mark no for those.

Right now I have:

Type of applicant: Individual

Part I Q1a DCN 64

Part II Q4 No

Part II Q13 No

Part IV Q26 Zero

Is this sufficient? If no can someone please assist with what additional questions I should fill out? As of right now, outside of the four questions above and my name and additional details on the very top of the form nothing else has been filled out.
