Did you use the Online version or the Desktop program? You have to go slow and watch what you click on. People get going too fast at the end and keep clicking on the bottom right button to continue. But that screen can trip you up. The bottom right button is to Pay with Refund. You need to click on the left button to pay with a credit card and avoid the Extra $40 Refund Processing Service charge (45 in California).

You can call customer service and pay with a credit card and then you will not pay the $40 for paying with your refund.
