Hello,

Do I have to fill out both YOUR PREMIUM % and YOUR SLCSP PERCENTAGE? I share a health policy with a relative that is not on my tax return. I can figure out my premium percentage but how do I figure SLCSP % or do I just leave it blank?

the same % must be used across the board for each month but different %'s can be used in different months. how did you manage to include in the policy someone that would not be included in your tax family.? don't bother to answer it makes no difference.
