    <description>&lt;P&gt;Hello,&lt;/P&gt;&lt;P&gt;&amp;nbsp; &amp;nbsp; &amp;nbsp;Do I have to fill out both YOUR PREMIUM&amp;nbsp; % and YOUR SLCSP PERCENTAGE? I share a health policy with a relative that is not on my tax return. I can figure out my premium percentage but how do I figure SLCSP % or do I just leave it blank?&lt;/P&gt;</description>

      <description>&lt;P&gt;the same % must be used across the board for each month but different %'s can be used in different months.&amp;nbsp; how did you manage to include in the policy someone that would not be included in your tax family.? don't bother to answer it makes no difference.&lt;/P&gt;
