      <description>&lt;P&gt;Which one?&lt;/P&gt;&lt;P&gt;Mine has the following &amp;amp; doesn't object.&lt;/P&gt;&lt;P&gt;(But not sure al 1099-R forms come from the same place)&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;Covington, KY&lt;/P&gt;&lt;P&gt;41015-1987&lt;/P&gt;&lt;P&gt;_______________&lt;/P&gt;&lt;P&gt;The other thing is to doublecheck the state selected.&lt;/P&gt;&lt;P&gt;__________________&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;</description>

      <description>&lt;P&gt;see if you can delete the field and manually enter just the first 5 digits. sometimes an invisible inbedded character is there which makes the field invalid. it happens from time to time when items are imported or maybe the dash is missing if there are the full 9 digits. at worst, delete the form and type in manually.&amp;nbsp;&lt;/P&gt;</description>
