Has anyone heard of a form 500 for dependent proof?

There is no such form needed on a tax return for proving the dependent can be claimed on a tax return.

There is a Form 500 but that is a Georgia state income tax return.

Where are you seeing this and what exactly are you trying to do?

The tech college we are applying to is asking for proof of presence. She doesn't pay any bills, she hasn't had her license for over a year so they asked for this 500 form. How can I aquire it. What and who do I contact for it?

You will need to go back to the school and find out Exactly the form they say you need. Have them show you a copy of the form and where to obtain this form. It is NOT an IRS form.

Proof of presence would not be a tax form you would obtain from the IRS or a State.

It is likely that Li-Madhu is correct and that the college wants a copy of her Georgia tax return, to show the address on it (as "proof" of residency). You can see a blank copy of this form here.

If they want something else, come back and tell us.

The admission office said it would be part of my 2021 filing to show proof of full time residency. Where would that be in my parents return.

If you were claimed as a dependent on your parent's tax return, your name and Social Security number is entered in the Dependents section on page 1 of the Form 1040.

There is no specific residency time noted on a federal tax return.
