All those fields Do exist on an IRS Form 1099-R.

Go to this IRS website for the 2022 Form 1099-R - https://www.irs.gov/pub/irs-prior/f1099r--2022.pdf

 

Make sure that you are using the correct section of the TurboTax program -

 

To enter, edit or delete a form 1099-R -

  • Click on Federal Taxes (Personal using Home and Business)
  • Click on Wages and Income (Personal Income using Home and Business)
  • Click on I'll choose what I work on (if shown)
  • Scroll down to Retirement Plans and Social Security
  • On IRA, 401(k), Pension Plan Withdrawals (1099-R), click the start or update button

Or enter 1099-r in the Search box located in the upper right of the program screen. Click on Jump to 1099-R

If you do not have those boxes or amounts on your 1099R just leave them blank on the screen.  Not every box is needed.

Some payers issue what is called a substitute Form 1099-R, one that is not printed on the official IRS Form, that omits boxes that have no entries.  As VolvoGirl said, just leave those boxes blank on TurboTax's 1099-R form.
