Where do I report foreign pension plan growth where no distributions or deposits were made (i.e. just appreciation)?

You would not report that anywhere.  The only action reported in relation to pension accounts is the distribution of income to the account holder or contributions made by the holder.  If you are taxed on appreciation in a foreign country, you would receive no foreign tax credit or deduction because you can only receive that credit or deduction for the same amount of tax that would be paid on that income in the US, and the amount paid in the US would be zero.
