Where do I add information on rolling over my 401K into an IRA and withdrew $10,000.

I added the 1099-R for the 401K but not giving me an option where I started an IRA and withdrew.

You only enter the 1099R.  Did you take the 10,000 out before you put it in the IRA or take it out of the IRA afterwards?   You might need a 1099R from the IRA for the withdrawal.

What code is in box 7?  After you enter the 1099R it will ask what you did with it. Then how much you rolled over.    Just keep going and answer the questions.  Read the screens slowly.   You don't need to enter starting the IRA.

Principle sent the full amount the to the IRA investor and they gave me the 10k. 

Box 7 code is G.

It didn’t give me that option to say where the investment went.

G is a direct rollover.  You need a 1099R from the IRA for the withdrawal.  Did you get one?  Ask the IRA plan for another copy or maybe  you can log into your IRA account and download the 1099 pdf.
