Found the solution. It appears I inadvertently replied no to a question did you receive any railroad pension or SSA income. Thus TurboTax removed the form. To reactivate the form, on the left side there is a clear form option. Once cleared, I could answer YES and then enter SSA or social security income.

To enter Form SSA-1099-SM (Social Security benefits) in TurboTax Mobile, go to the Federal section and then select Income.
Look for Retirement Plans and Social Security and choose to Start or Update Social Security (SSA-1099) entry.
Answer “Yes” if asked about receiving Social Security benefits, then check the box for the form SSA-1099-SM, and enter the amounts exactly as shown on your form.
If you see only a Revisit option that loops you back, try closing and reopening the app or refreshing your session. This sometimes clears the navigation issue.
Make sure you are fully through all prompts in the Social Security section to complete the entry.
IF it keeps looping without allowing you to enter any data>....
A very few other folks have had this issue....the fix for them was the following.
For Online tax Software:
1) go into the Tax Tools menu in the left side.
2) find the Delete a tax form in the pop-up menu
3) Delete the Social Security Benefits Worksheet
4) Return to the interview and you should now be able to enter your SSA-1099 information.

For Desktop tax Software:
1) Open your tax file and switch to Forms Mode
2) Find and click on the SS Benefits Wks in the left-side window
3) Delete the Social Security Benefits Worksheet in the Rt-Side screen
4) Return to the interview and you should now be able to enter your SSA-1099 information.
