You might want to go back and edit this 1099-R to go through the followup questions again. It seems you indicated that the distribution was for more than one person. Better yet, delete the 1099-R and enter it again.

Is there a way i can pull up the forms i need to send to my city? when i filed it wouldn't let me print or save them so i tried to proceed and go back later.
