What paperwork do I need to file pulling from IRA for COVID Relief

I'm needing to pull from a traditional IRA for relief due to COVID what paper work do I need to submit? Also how to ensure the income is distributed over 3 years?

2020 TurboTax will be implementing Form 8915-E for reporting Coronavirus-Related Distributions, but that form is not yet present in TurboTax.  That form is presently only available in draft form from the IRS, but you can look at that to get an idea of how your CRD will be reported:

That will be part of the 1099-R interview and will go on a new 8915-E form that the IRS has not released and is still in the draft state at the IRS.  It will be at least 2 months before the IRS releases the electronic specs of that new form.   New forms and tax law changes are often delayed far into the tax season.
