You'll get a 1099-R if you received $10 or more from a retirement plan. Typically the payer is the name of the Brokerage Firm, Bank, etc where your retirement funds are located or managed. The payer's city is the city where the Brokerage firm is located.
If you have a Form 1099-R, the Payer's name, address, city, state and zip can be found in the top left corner of the form. If you don't have or need to report the 1099-R, see the steps below to delete the form.
