If you can claim your parent as a dependent you enter the parent as a dependent in My Info. You will get the $500 credit for other dependents for claiming them. You will need your parent's SSN and date of birth.

Q. What form do you get to fill out if you claim your parent on taxes?

A. None. You parent's name, SS#, and relationship are only entered in the "Dependents" box on the top part of form 1040. In TurboTax, enter as described by @xmasbaby0 in the other reply.
