What do I do if there is an amount in Box 4 of my SSA-1099-SM form? Is it entered anywhere on my return?
No, that isn't entered anywhere on your return. All you enter is the amount that is in Box 5, any medicare pt b or d withholding and any taxes that you may have had withheld.
