You can answer Yes to the question asking "Did you put money into your HSA?"  

However, do NOT reenter the contributions that were already accounted for on your Form W2.  See screenshot below.  Those contributions should get pulled into the top line and you would only enter something on the second line if you made contributions outside of your employer.

Because my plan is a café 125, my $1500 contribution is included as part of the contribution showing in box 12.  Box 12 is for employer contributions and employee contributions to this type plan.  So, the question is, is my contribution really my contribution?

If the answer is yes, then I get a deduction.  If no, then I don't.

I should add that the letter code in box 12 is W

Contributions made through your payroll are technically considered employer contributions (even your portion) and all of these contributions are reported using Box 12 Code W.  You are already receiving the pre-tax benefit of the deduction by having them deducted pre-tax from your payroll.  So, you are getting the deduction - just through your payroll and not on the tax return itself (its actually a better deduction because you save on FICA taxes).

You would only enter additional contributions as "your contributions" if you made an additional contribution outside of your employer (i.e. directly to your HSA from your personal bank account).  In this case, you would then add it to that screen above and get your deduction on the tax return itself.
