It depends upon the state in which you live. Post back to THIS thread if your state's taxation department doesn't have that information.
For federal tax:
If it was a Roth IRA, then No.
If it was a Traditional IRA, then Yes.
If it was taxable, then you should have gotten a 1099-R. The code in box 7 of that form will tell you whether it's taxable or not.
