Took early distro from 401k in '16. Paid $828 in federal tax. 1099-R shows $0 in federal tax withheld section. How do I get credit for $828 I paid & avoid addtl tax due?

How did you pay the tax?  If it was withheld from the distribution by the trustee, you need to get the plan trustee to issue a corrected 1099-R.  There isn't any other procedure that wouldn't result in an IRS letter and bill for taxes due.

It was withheld by the trustee (Fidelity). I intentionally had them withhold double the minimum amount so that I could avoid a hit when I filed taxes. I will definitely call them and see if there's an error. I've never had to file a 1099-r before so didn't know if this was common. Thanks!
