The State of Nevada is not listed as an option in Box 15- State on your 1099-R form. I have NV in this box on the 1099-R I received from Fidelity Investments.

You can ignore the NV on the form and leave it blank. Nevada doesn't have an income tax.

NV has no state income tax so it will not appear in certain drop down menus. Just leave the spot blank.

Since your state does not have an income tax then leave all boxes 14-16 Blank.
