The IRA/SEP/SIMPLE box is not checked and there is not a FATCA box on my 1099. I don't understand what lines 8 through 11 are.

Enter the Form 1099-R exactly as received.  If the (substitute) Form 1099-R that you received omits certain boxes, leave them blank on TurboTax's form.
