The financial institution files Form 5498 with the IRS for each account, but is only required to provide a copy of the Form 5498 to the participant if there were contributions (including rollover contributions, conversion contributions and recharacterization contributions) to the account for the year. Otherwise, they are only required to provide on a year-end statement the year-end balance and an indication as to whether or not an RMD is required.
