You may have picked the wrong kind of 1099R.  Delete this one and start it over.

The Boxes on the 1099-R and Turbotax don't align

Is it a CSA or RRB 1099-R?

Be sure to pick the correct 1099-R type: Standard 1099-R, CSA-1099-R, CSF-1099-R, RRB-1099-R.

I did further forensics and was able to determine the solution.

The Boxes on the 1099-R and Turbotax don't align

From OMB?

....that's a CSA-1099-R for the original recipient of the pension

...or a CSF-1099-R if it's for the survivor of the original recipient.

Form 1099-R from OMB

If from OMB then it is either a CSA 1099-R or a CSF 1099-R.   The CSF is for an inherited account.   Delete and reenter.
