The amount of health insurance premiums shown om my 1099-R and the amount shown deducted from my retirement benefit are not the same. Which should I use?

Use what was actually paid regardless of what is on the 1099R.

Some for example only include the health insurance premiums and do not add the vision and dental insurance and both of these are also medical expense premiums.

When you use a different amount from the 1099R just make sure the tax file you have has a note as to how the deduction was arrived at.
