Should I be receiving a form from my former employer showing what months I was in their company health ins. plan for in 2016?

I left the company 9 months into 2016 and didn't have 3 months of ins. The ins. was not affiliated with the ACA.

Yes, you should receive a 1095-B or 1095-C, depending on the size of the company.  But you don't need to enter either of those forms into TurboTax; they're for your records only.

What's important here is that you answer the first question in the TurboTax Healthcare section, which asks whether you had coverage the entire year.  Select the box that says you'll enter the months you had it, and then you'll be shown the monthly boxes to check.

When you continue, TurboTax will ask you some questions to determine if you qualify for any exemptions for those three months without coverage.
