Rollover from former employer 401k to a traditional IRA.

I did a rollover from former employer 401k to a traditional IRA. My institution did not send me any 1099-R, only a 5498. Do I need to report anything? Turbotax seems to be asking only about 1099-R (which I don't have) and ignores the 5498...

The 5498 is just for you to keep for your records.  Your rollover directly from one institution to another does not need to be reported as it would be tax free, as long as you  took no money out.
