received 1099-r from my old employer from my 401k. Box 7 is checked "G" a direct rollover but not the box IRA?SEP?SIMPLE. should it be if the money came from 401k?

No. If the funds being rolled over came from a 401(k) account, the IRA/SEP/SIMPLE box should NOT be checked.
