That would be odd.   Have you phoned the Payer to inquire why it's not on there?

Is it a standard 1099-R, such as this example for 2021?

Ask the payer to send you a 1099-R that is filled out correctly.

Did you get multiple copies of the 1099-R? If so, do any of the copies have the payer's TIN?
