'ENTER YOUR 1099-R PROVIDER'S INFO'. UNABLE TO ENTER US ADDRESS

Make sure you select "US Address" as the address type and enter the information.

If that doesn't work delete this Form 1099-R and try to reenter it:

1. Login to your TurboTax Account 
2. Click on "Search" on the top right and type “1099-R” 
3. Click on “Jump to 1099-R”
4. Delete Form 1099-R
5. Click "Add Form 1099-R"
