On my W-2 form it keeps on saying needs to be reviewed from questions 15-20. I have added everything that is on my W2 form for those specific boxes. What could be wrong?

Yes, it is referring to the entries of the state and local tax authorities id #, names, wages and withholding for all 6 boxes.

Be sure there are no extra spaces in the beginnings of any of those boxes because it could cause an empty field to be assumed.
