Leave box 11 blank.  Box 11 only applies to distributions from Roth accounts in qualified retirement plans (code B or H in box 7).

On your 1099-R, since the distribution is from a retirement account, the Code for Box 7, should be Code 7.  This is indicating that it is a normal distribution.  The distribution is taxable on your Federal tax return.

Since this distribution is from a retirement account, Box 11 should be blank.
