Form 1099-R 2020 is slightly changed and TurboTax use the newer version. It looks like the financial institution which issued your 1099-R used the old format for some reasons. It shouldn't create a problem for you, just enter your amounts to the correct boxes.
Either:
1) You actually have a CSF- or CSA-1099-R and are entering it as the wrong form type
or
2) Your provider failed to update their forms to the proper new 2020 format (who issued that form??)
or
3) you are attempting to enter a 2019 1099-R form into the 2020 software.....can't do that, a 2019 1099-R form has to go into 2019 software.

If #1, work your way out an delete that form, then start a new 1099-R, but carefully select the proper form type on the page that lists the 4-various form types you can enter
If #2, call the provider and insist they re-issue their 1099-R forms in the proper 2020 format.

I'm going through the final stages of review on the return and TurboTax is asking to verify the 1099R form. Unfortunately, the provider (a state agency) is still using the old form. there is an amount in box 12 for swt. without reaching out to the state for an update, what would be the best recommended step forward?
You just translate amounts from your paper form to the proper boxes in the 2020 software.

State ID and ID number in box 15
State distribution in box 16
.....................(IF no state distribution shows on your 1099-R form, some states require you to put the value of box 2a into box 16 in the software .....or the federally taxable amount of box 1 if box 2a is empty)

I had the same question and I imported the information as if it was the new form. I put the information from box 12 into Turbotax box 14. My return went through fine!
