On form 1099-R the Payer's address is not listed. How or where can I get this information as it is required to Efile according to TurboTax?

The 1099-R should include the payer custodian's own Tax ID number.  As with your own 1099s or W-2, adding this number into your TurboTax page will likely allow TurboTax to auto-populate the requested information for you.  You can also search the payer on the web, find a web page for that custodian and use the "Contact Us" link to reach them and get questions about your account answered - including their address.
