OMB 1099-R 2a is unknown, no 2b, fed tax pd is in box 4, box 7 is #4 - death. I don't know how to answer 'What did you do with the money from OPM?' this is an annuity.

If you did not roll over the amount to another retirement account, you should answer that you did something else with the money.
