My former fund manager firm rolled over my Roth IRA funds to my new Roth IRA fund manager firm but never sent me a 1099-R which I now belatedly realize I needed to declare the transfer on my tax return. These are all after-tax. I am NOT retired but have not contributed to Roth IRA in over a decade. I will try to contact my old firm tomorrow and hope they can or will help me. If not, I will do the best I can. For my situation, under box 7, is code H - direct rollover of a designated Roth account distribution to a Roth IRA correct? Also, any tips in filling this out is appreciated. TIA.

Your Roth IRA was apparently moved by nonreportable trustee-to-trustee transfer, the proper way to move an IRA, not by distribution and rollover. Nothing about a trustee-to-trustee transfer is to be entered into TurboTax.
