My wife turned 70 1/2 in 2017. She took her RMD for 2017 in 2018 (before April 1). How do I report her RMD without a 1099-R?

It won't be reported until she files her 2018 return in early 2019.

Note that she must take 2018 RMD by 12/31/2018, so she will be reporting both of them on the 2018 return.
