My wife took her entire 2016 RMD and asked Vanguard to send a check to Doctors without Borders so she could use the RMD for charitable purposes and not declare the income
However her 2016 Form 1099-R indicates the entire amount as taxable income. How do we show on Turbo tax that she did not receive these funds

After you enter your 1099-R , you will reach a screen: Transfer to Charity? where you can indicate that all or part of the distribution was directly transferred to a qualified charitable organization. See the screenshot below.
