My NY tax is zero! Why is line 3 in Part II of Pen Ann IRA Wks blank and instead the expected amounts appear in lines 2a and 2b?

I checked that the page for 1099-R entries is correct. 

Last year's worksheet had the numbers in the correct places. and the calculated tax was consistent with previous amounts.

With guidance from a telephone support person, I learned that the problem was caused by failure to designate the proper exclusion category in the Fed. part of the return.
