My husband rolled over 401k to Ira. We took 5,000 and put rest into the Ira account. How do I put into the taxes the 5,000. Morgan Stanley doesn't give 1099.

I don't know where to claim the $5,000 that we took out of the IRA.

It is really better to have a form 1099-R, but if the company doesn't issue one you'll need to prepare a substitute form 1099-R. Please follow the directions below to do so.

1. Click on the federal taxes tab, at the top of the page. (the personal tab in Home and Business)

2. Click the income and expenses tab. (personal income in Home and Business) 

3. Click skip to see all income.(if shown)

4. Click the show more link, next to retirement plans and social security.

5. Click the start tab next to IRA, 401(k), Pension Plan Withdrawals (1099-R).

6. select the tab to add a 1099R.

7. Click "I'll type it in myself".

8. On the next screen, select, I need to prepare a substitute 1099-R.
