My 1099-R is reflecting my old address and state ID since I moved halfway through the year but my distribution is taxable in my new state. Will I need to get a new form?

Make sure that the issuer knows your new address.

You can still file the return with the current 1099R. 

Yes, you are correct the 1099R is income to the state you were living in at the time it is received.

You should be able to do an allocation when you get to the State(s) to determine how much was received in each State.
