Code 7 and code B are two separate codes and both codes must be entered. See the answer to your duplicate question.
@AmyC , this was apparently an indirect rollover of a Roth 401(k) to a Roth IRA, so Code 7 combined with code B is correct. Code H is only for direct rollovers. (Even if it was actually a direct rollover, codes 7 and B together will still produce the correct result when reported as having been rolled over to a Roth IRA.)
