Your broker has issued that statement as a 'substitute' for the necessary 1099-B form. It should still report whatever stock transactions you may have had as well as what the basis was and whether it was reported to the IRS.
In other words, all of the information necessary to answer the questions regarding the stock trades you had for the year should be contained on that form.
If you are having trouble reading it then you may need to reach out to the broker who issued it to you for a little guidance.
