Mother is 93 receives SS and 3000. on. 1099R. Does she have to file income tax?

If that is all the income she has to file, then no she does not have to file a tax return.

Her standard deduction is $12,000 which would wipe out the $3k and her SS is not taxable at all with only the small amount of additional income.
