Why does line 16a not match 1099R taxable amount line 2a?

Line 16a of Form 1040 is the gross amount of "Pensions and annuities" income, not the taxable amount (shown on Line 16b).

Line 16a should show the total of your 1099-R Box 1 amounts; line 16b should show the total of your 1099-R Box 2a amounts.
