Form 5498 isn't required to be sent until May 31, 2018. This is because a taxpayer can still make IRA contributions up to the due date of the return for the prior tax year.
The form is used to record annual contributions to an IRA. The form is not entered on your tax return, but keep it for your records. It comes into play when IRA basis is needed.
You can use your year end statements for your IRA if asked in the program for the value.
