It's asking for info from at 1099 form but I only got a 1095 form from my employer

A Form 1095 tells you if you were covered by an insurance plan at work.  You need that in addition to your 1099 to file your 2016 tax return.

Your 1099s don't have to be out until January 31, so you still have a few days to wait to receive it in the mail.
