IRS asking for completed 5329 for amended return but 5329 wouldn't be there in the first place, what do I need to submit?

I have made a mistake last year and reported my HSA contributions twice - form 5329, line 47 and 48 filed as excess contributions (the contributions counted twice). Submitted 1040X and now IRS is asking me to submit a completed 5329 which wouldn't be there in the first place. Should I just sign a form with nothing on it or specify 0 on lines 47 and 48 - not sure what to do here... Thanks for the help in advance!

Yes, please include a signed 5329 form.  Fill in Section VII with zeros and add a letter stating that you reported your contributions twice in error.

As DianeC958 indicated, you need to put zeros on lines 47, 48 and 49 of Form 5329 to explicitly correct the incorrect amounts on your originally filed Form 5329.  You'll need to do this manually because TurboTax will not automatically include the zeros.  If you are amending your 2018 tax return you are presumably using the CD/download version of 2018 TurboTax, so you can do this in forms mode by overriding line 47 and entering a zero, then explicitly printing this form, the corrected Form 8889 and Form 1040-X.
