Enter the amount in Box 12.  W is the code (employer contribution to Health Savings Account), which you enter in the box next to the amount.

In the W2 entry screen in Box 12.  You don't enter it again anywhere else.

Thank you.  Is the same amount suppose to show up on line 9 on form 8889?

Yes, usually.  However, it's uncommon, but amounts reported with code W sometimes include amounts contributed for the prior year which should have already been reported on the prior year's Form 8889.  Your employer would/should have told you if any portion was a prior-year contribution.

Thank You.  For 2016 Line 2 on the 8889 form is 0 however line 9 is completed.  Do I need to correct 2016?

You haven't mentioned anything that would require filing an amended tax return.  Without a reason to believe that the 2017 code W amount represents contributions for any year other than 2017, the entire amount should appear on line 9 of your 2017 Form 8889.
