If I have a year end statement, not a 1099DIV or 1099 INT and the statement shows ohio tax free income fund, where do I enter this on my federal return

I have a year-end statement the fund name is Franklin Ohio Tax-Free Income Fund Class C The year end summary shows the following.

Tax-exempt income Dividends: $500.00

Long Term Capital Gains           $.00

Do I report this form on my federal return, and where?

Do I report this form on my State return and where?

No. You don't report it anywhere, which is why you didn't get the 1099-DIV.

From the Franklin site: "That fund seeks to provide investors with as high a level of income exempt from federal income taxes and Ohio personal income taxes as is consistent with prudent investment management and the preservation of shareholders’ capital.
"

No. You don't report it anywhere, which is why you didn't get the 1099-DIV.
