If I consolidated a few ira/401(k)/401(B) into a new traditional ira should i have received 1099 R forms for all?. Only received them for the 401(k) and 403(B).

A IRA to IRA trustee or account change is not normally reported in a 1099-R if a trustee-to-trustee transfer. 401( ) type rollovers must always be reported on a 1099-R.
