I withdrew from life ins policy as retirement income. The 1099-R reflects the withdrawal, but didn't reflect the cost basis. I have the cost basis, where do I enter it?

The insurance policy was purchased with income that had been taxed. The insurance policy has been held to 10 yrs. The 1099R from the Insurance company did not include the basis.

Is the taxable distribution the same as the gross distribution?

If the payer returned part of your cost basis then they should account for that in box 2a.  If you have questions about that then contact the payer.
