I withdrew money from my IRA and received a 1099-R. I repaid the full amount well within the 60 day limit. My financial consultant said there is no form created for repaying an IRA withdraw. I have a copy of the statement showing the date of the withdrawal and the repayment.

What you did is called a rollover. After you enter the 1099-R for the withdrawal, continue going through the questions in sequence. You will get to a screen that asks "What Did You Do With The Money" from the IRA. Select the choice that says you "moved the money to another retirement account (or returned it to the same retirement account)." It will then give you three more choices. Select the one that says you "rolled over all of this money."
